Release Form

I, hereby authorize Proud Times permission to use my likeness, submitted video media, and submitted original artwork in any and all of its publications, including but not limited to all Proud Times’ printed, social media and digital publications. I understand and agree that any image submitted using my likeness or original works created by myself will become property of Proud Times and will not be returned.

I acknowledge that since my participation with Proud Times is voluntary, I will receive no financial compensation.

I herby irrevocably authorize Proud Times to edit, alter, copy, exhibit, publish or distribute the submission of my likeness, submitted video media, and submitted original artwork for purpose of publicizing Proud Times’ programs or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, where in my submission of my likeness, submitted video media, and submitted original artwork appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my likeness, submitted video media, and submitted original artwork.

I hereby hold harmless and release and forever discharge Proud Times from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I am at least 18 years of age and am competent to contract in my own name. I have read this release before signing below and I fully understand the contents, and impact of this release.
